Dear patient, because we collect numerous attempts to outstanding debt, we believe it is in the best interest of both parties, that our doctor-patient relationship must be terminated on that date. Note that a period of thirty days are in a different contact list provider.please dentist has received, if any questions or concerns you arise.thank ...

Keep it simple.
This letter will inform you of your last notice of non-payment of amounts due for services rendered to () to his office. If not paid within 30 days after the date of postmark, will declare thy name of a collection agency and its continued decline will be respectfully delivered. After this point, we do not assume any liability for further assistance in your dental health and wellbeing.

I just want to say that your account was sent to collections due to a gap in accountability (I am sure that they have signed some kind of form, if the treatment has started ...) and also say that because it has a collection account can no longer treat their installation, but I would be glad to submit their letters and history in his new dental practice.
